NEW Regional Forms for Junior Division Grades 6-8
Links to the Forms are below; please read first

1. Did you answer the four (4) questions from the Pre-Experimentation Review Questionnaire? If not, do that first by clicking Online Eligibility Forms. This will determine if your project requires "pre-experimentation review" or is "exempt" from pre-review.

2. If your project requires pre-review, the forms below replace the ISEF forms previously used for grades 6-8. Students exempt from pre-review will also need to submit the Checklist and Research Plan if they advance to the regional fair.

3. The regional junior division forms require Adobe Reader. When opening the file, if your Adobe version gives you the option, check the HIGHLIGHT FIELDS BOX and this will highlight all of the fields that it will automatically tab to.

4. Students may print forms and hand-write information or complete them online, save and print. NOTE: When saving, change the file name to prevent overwriting especially if computer will be used by more than one student.

5. Regional forms must be printed and signed before submitting for pre-review.

1.) REGIONAL CHECKLIST FORM:
This Checklist form is required for all pre-review projects and for exempt students that advance to the regional fair. The student, adult sponsor, and parent all sign this form. (We recommend that exempt students fill it out and get it signed before they begin their project. In the event they advance, their form will have proper signatures and dating. Exempt students do not need to mail the exemption form to IPFW unless they advance to the regional fair.)

2.) RESEARCH PLAN:
We have included an optional research plan guide with the checklist. If your teacher requires a written research plan, then this form will not be needed. If your teacher prefers, complete this form instead of a separate research plan. CHECK WITH YOUR TEACHER TO SEE IF YOU SHOULD USE THE REGIONAL RESEARCH PLAN FORM.

3.) PRE-REVIEW PROJECT FORMS:
For pre-review projects, submit the Regional Checklist, Research Plan, plus the form below that applies to your project before the Jan. 28, 2009 deadline.

  • Human subject forms grades 6-8
    For projects using humans as subjects
    We've included a user-friendly human subject consent form! NOTE: Consent forms DO NOT get signed by the research subjects and/or their parents until AFTER the project has SRC approval. The SRC will indicate if written consent will be required.


  • Vertebrate animal form grades 6-8
    Used for projects involving vertebrate animal research

  • Risk assessment form grades 6-8
    Used for projects with potentially hazardous biological agents and/or hazardous chemicals, devices and activities.
    If your project is exempt from pre-review but uses hazardous chemicals, devices, or activities, this risk assessment will be needed for post-review by students that advance to the regional fair.
